Does this mean you need to hand draw the map, or can you cut it out of a printed map? Does it have to be a real place, or can it be a place from a dream or a book?

You can cut it from printed, it can be real or imaginary. Just make sure you put the meaning and where it is on the back (especially if it's imaginary).
